The review is complete. Both findings are written to
.imd-findings.jsonand anchored to verified lines, and the working tree holds no other changes.Outcome. The diff between the two commits is sound in its main claims. The Etherscan cap is reserved before the send under a revision check, failed GUIDs are retained as attempt rows, the 409 memo suppresses only re-upload, stale errors clear on a new GUID or a pending or pass status, and the jobs cursor now rotates fairly. I found two defects, both low severity, both reproduced with scratch tests run against the tree.
Finding 1: operator-side refusals burn the unrecoverable cap at
adapters/source-verification.mjs:62. A status zero body such as "Invalid API Key" or "Max rate limit reached" is Etherscan saying no GUID was issued, yet it still counts as one of the three durable submissions. Three such replies put the row in operator_action_required within three minutes, and repairing the key never restores it. The scratch run showed zero real submissions ever reached Etherscan after the key was fixed.Finding 2: a race drops an accepted GUID at
adapters/source-verification.mjs:36. If a second step for the same address runs while the first is parked on a slow POST, the second step's final save makes the first step's post-submission save stale. The accepted GUID is discarded, no attempt row is written, the reservation stays charged, and the next step resubmits the identical input. This is reachable because the 120-second lease is shorter than the worst-case provider time inside one step. What I checked and found clean.
- Sourcify's upstream source confirms a 409 means only "already verified and not improvable", while duplicate in-flight requests are a 429, so the memo cannot mask a transient failure.
- The cursor rework passed randomized sweeps over 1,000 to 3,001 rows with a stale legacy cursor, and a four-job rotation case.
- The full suite in a scratch copy with dependencies installed passed 433 tests with 10 skipped. Doc regeneration produced no drift from the committed docs, and the version is consistent across package, lockfile, interface and OpenAPI.
- Every caller treats the new stale error as a waiting retry, not a terminal failure.
Not covered: no live chain or explorer calls were made, and the Postgres store path was reasoned from source only.

Review complete. The findings file is written and validated, and the tree is unchanged apart from my scratch directory.
Outcome. The candidate fixes the four prior findings as described, and the claim-cursor change holds up: a randomized property test over 2,600 rows with mixed terminal and live jobs claimed every available job in exactly one poll each, with no starvation at any cursor position. The economic regression is in how the new submission cap is debited.
Findings written to
.imd-findings.json:- Medium, source-verification.mjs line 59. Etherscan replies that explicitly refuse a POST (HTTP 200, status '0') are debited against the durable three-submission cap. With the callers' 30 s re-step and the 60 s cooldown, the three submissions land at 0, 90 and 180 seconds after confirmation. An explorer indexer lag or rate-limit window longer than three minutes exhausts the row with zero accepted submissions, and nothing can submit again. The module then stays 'verifying' forever,
prepareRegistrationrefuses it, the deterministic module id blocks redeploying the same reviewed case, and sponsor gas is already spent. The deployed runtime retried these refusals until acceptance, so this is a regression. A node proof test that fails on the current code is attached. - Low, chain-deployments/index.mjs line 169. An exhausted verification is re-stepped every 30 s indefinitely with two explorer calls per step. Enough stranded rows exceed the shared daily Etherscan quota, after which every new verification is refused and exhausts via the finding above, a self-reinforcing cascade.
- Info, source-verification.mjs line 32. The legacy migration counts a row as one submission, but the deployed verifier re-POSTed after every refused reply, so some legacy rows carry more than one real submission.
Verification. The two changed test files pass. With dependencies installed temporarily, the full suite ran with 433 passed, 10 skipped, 0 failed. Sourcify's published spec confirms 409 means only
already_verified, so that part of the change is sound. Legacy-row migration undercounts: the deployed verifier re-POSTed after every refused (status '0') reply, so a legacy row may carry several real submissions but is migrated as oneadapters/source-verification.mjs:32
The migration assumes 'at most one original submission per row'.
That holds for accepted submissions (the pre-12ddbe0 code never deleted etherscanGuid), but the deployed code (git show 12ddbe0^:adapters/source-verification.mjs) resubmitted whenever a POST returned status '0' (no GUID stored, only etherscanAttemptAt refreshed) after the 60 s cooldown, so a legacy row whose POSTs were refused N times holds etherscanAttemptAt and etherscanError with no GUID and N>1 real submissions.
Such rows are migrated as count 1 and get two more submissions. The impact is bounded (at most two extra identical-input POSTs per legacy row, no payment or transaction), so this is informational, but the stated migration invariant is not exact.
State: row written by the previous runtime {etherscanAttemptAt: T, etherscanError: 'Unable to locate ContractCode at 0x..', no etherscanGuid, no etherscanSubmissionCount} after three refused POSTs.
Call step(input) with now() > T+60000 and the explorer still refusing.
Expected per the stated invariant: the row is treated as already having consumed its budget proportionally.
Actual: etherscanSubmissionCount becomes 1 then 2 after this step; two further POSTs are sent before exhaustion.
- Medium, source-verification.mjs line 59. Etherscan replies that explicitly refuse a POST (HTTP 200, status '0') are debited against the durable three-submission cap. With the callers' 30 s re-step and the 60 s cooldown, the three submissions land at 0, 90 and 180 seconds after confirmation. Scope reviewed: the diff from commit 12ddbe0 to 12da8a6 (the Etherscan submission cap with revision CAS, Sourcify 409 handling, stale-error clearing, and the per-claim job-scan cursor), plus the immediate callers in chain-deployments, chain-executor, vault and wallet executors, and both store implementations. The 12 verification cases and 6 job cases pass locally. Regenerated docs match the committed ones. My own randomized scratch tests confirm the new cursor always reaches a claimable job within ceil(N/1000)+1 scans and rotates waiting jobs round-robin across a 2,200-row history, so the starvation fix holds.

.imd-findings.json:- Medium, source-verification.mjs:62. A definitive non-submission reply from Etherscan (HTTP 200 with "Invalid API Key", "Max rate limit reached", "Daily limit reached") still spends one of the three permanent slots. Reproduced: after the key is repaired, zero further POSTs ever happen and the row stays
operator_action_required. - Low, source-verification.mjs:36. The new revision CAS discards an accepted GUID when a second step of the same row interleaves between the reservation save and the post-POST save, which is reachable because five 25 s explorer calls can outlast the 120 s lease. The count stays right, but the GUID is never persisted or recorded in the attempts history, and a duplicate submission follows after the cooldown. Reproduced.
- Low, source-verification.mjs:32. The legacy migration counts a row as one submission, but the deployed verifier re-POSTed every 60 s whenever no GUID came back, so a legacy row with repeated refusals can carry many real submissions and still gets two more.

Review outcome
The findings are written to
.imd-findings.json. Two medium findings and one informational note, all in the Etherscan submission-cap logic of the verifier adapter. The jobs cursor change and the version/docs synchronisation came through clean.Medium: not-yet-indexed explorer refusals exhaust the durable cap. The reservation at
adapters/source-verification.mjs:62counts every non-success POST identically. Etherscan replies "Unable to locate ContractCode at 0x..." until it has indexed a deployment, and the executors call the verifier right after 2-block confirmation with a 30-second job retry and 60-second cooldown. If indexing lags more than 3 minutes, the row hitsoperator_action_requiredand never submits again, even once the explorer would accept the identical input. I reproduced this against the pinned adapter and against the pre-change adapter with the same mock: the old code recovered with a fourth submission, the new code is stuck. Foundry's own verifier treats this exact reply as a retryable "could not detect deployment".Medium: an accepted GUID is lost when another step saves during the POST window. The slot is reserved in one CAS write, then the GUID is persisted in a second CAS write at
adapters/source-verification.mjs:73. Every step ends with an unconditional save, so a purely read-only step that interleaves with the up-to-25-second POST bumps the revision and the GUID save throwsverification_step_stale. The row then holds a consumed slot with no GUID, no error and no attempt record, and the next step resubmits the same source. The authors' concurrency test starts all steps at once so this interleaving never occurs. Overlap is reachable via lease expiry mid-step with a second worker instance, or an operator running the mainnet release script alongside the worker.Info: legacy migration premise. A pre-0.12.7 row with
etherscanAttemptAtand no GUID could represent many refused resubmissions, since that adapter only stopped on a stored GUID. It migrates as one. The bound still holds, so this is a note on the stated premise, not a defect. What checked out clean.
- The per-kind cursor advance and single wrap: I model-checked six table geometries with 1,000 to 3,000 rows and random cursor positions. Every eligible job was found within ceil(N/1000)+1 claims, and 1,200 eligible jobs were each claimed once before any repeat. The
<=wrap filter, the1000 - records.lengthlimit and thenextCursorafter a wrap are all consistent. - Sourcify 409 handling: the live spec says 409 is "already verified" and "already being verified" is 429, so the suppression is correct.
- Version 0.12.8 is consistent across package files, the API contract and docs, and regenerating the docs produces no diff.
- The 18 focused jobs and verification tests pass locally.
Each reproduction in the findings file is a self-contained Node test that runs against this tree. No Foundry proofs were attached, since no Solidity code changed in this diff.

Legacy rows with etherscanAttemptAt but no GUID may already represent many refused submissions, yet migrate as oneadapters/source-verification.mjs:32
The pre-0.12.7 adapter (48c611a) only stopped resubmitting once a GUID was stored; a refused POST (status '0', for example 'Unable to verify') set etherscanAttemptAt and was re-sent every 60 s indefinitely. A surviving legacy row with etherscanAttemptAt set and no etherscanGuid therefore may stand for an unbounded number of identical submissions, not one, so the stated premise 'migration sees at most one original submission per row' holds only for rows that received a GUID.
The migrated count still bounds future submissions to two more, so this is informational; note it in the migration text or count such rows as 3 (already exhausted) if the intent is to never add to an already-refused legacy history.

- Medium,
adapters/source-verification.mjs:72. Explicit Etherscan non-acceptance replies ("Unable to locate ContractCode" during indexer lag, "Max rate limit reached", "Invalid API Key") are charged against the permanent three-submission cap. With the worker re-stepping every 30 seconds and a 60-second cooldown, the three POSTs land at 0, 90 and 180 seconds after confirmation. If the explorer still refuses at 180 seconds the row becomes operator_action_required with zero accepted submissions and never submits again, even after the explorer would accept. The module cannot be redeployed for the same case and a stuck registration blocks the project. This is a regression from 12ddbe0, which retried these refusals. Merged from four specialists. The attached specialist proof is a Node test, not Foundry, so it is referenced rather than attached. It fails on this code as claimed. - Low,
adapters/source-verification.mjs:36. A step that completes while another step's POST is in flight bumps the row revision, so the accepted GUID fails the CAS and is discarded. The slot stays spent, no attempt record exists, and the next step resubmits the identical input. Reachable when a step outruns the 120-second lease and the per-minute cron re-claims. Merged from three specialists. - Info,
adapters/source-verification.mjs:32. Legacy rows with refused history and no GUID migrate as one submission, though the deployed verifier re-POSTed every 60 seconds without a GUID. Bounded to two extra POSTs per row. Merged from three specialists.
Dropped. The claim that exhausted rows are polled every 30 seconds forever without backoff is true but describes unchanged caller cadence, and continued read-only polling is the stated design so manual verification can complete the job. I noted the quota cost inside the medium finding instead.
Checks run. The verifier and jobs suites pass on this tree, as does the new cursor fairness test. The full suite could not run because third-party dependencies are not vendored in the tree. My own review of the jobs cursor change found no regression. Scratch tests were removed and the tree is clean.
