6.lowOn-chain setPolicy allows delays up to 365 days and is monotonic, but proposal_create rejects delays over 5 days: one owner call irreversibly removes the project from reviewed upgradesadapters/chain-read.mjs:127
requireThat(s.minDelaySeconds <= 5 * 86400, 'unsupported_delay');
ClausManagedExecutor.setPolicy accepts delay <= MAX_DELAY (365 days) and never allows decreasing it (ClausManagedExecutor.sol:272). The network refuses any proposal for a project whose on-chain minDelaySeconds exceeds 432000. An owner calling setPolicy(id, v, 432001) directly (they own the registry entry) permanently loses network-managed upgrades; nothing documents the mismatch.
Owner-only and self-harming, so low.
Fix: align bounds (cap MAX_DELAY for policy at 5 days in the candidate registry) or document that a direct setPolicy above 432000 s is irreversible for network-managed upgrades.
Registered managed project (minDelaySeconds 0).
Owner calls setPolicy(projectId, keccak256('policy-2'), 432001): succeeds (432001 <= 31536000 and >= 0). proposal_create -> chain.proposal reads state() minDelaySeconds = 432001 -> requireThat(s.minDelaySeconds <= 432000) throws 'unsupported_delay'. setPolicy(projectId, v, 432000) now reverts InvalidPolicy (delay < p.minDelaySeconds).
Expected: bounds agree or documented; actual: permanent exclusion.
7.lowSepolia managed binding is written after 2 confirmations but managed evidence reads at the finalized block, so the first proposal fails for ~13 minutesadapters/managed-evidence.mjs:21

Registration confirmation for chainId 11155111 requires only head - receiptBlock >= 2 (adapters/chain-executor.mjs:119-120; finality only enforced for chainId 1), after which the binding exists. captureManagedState reads target code, executor() and projectId() at snapshot.blockNumber, which createChainEvidence takes from blockTag 'finalized' (adapters/chain-evidence.mjs:59).
Until the registration block is finalized the target has no code at that block, so proposal_create fails (viem zero-data error on target.executor() or managed_registry_changed), and managed_registry_changed is in the permanently 'blocked' list at src/approvals/index.mjs:196 if hit during decision. Transient; self-heals after finality.
Fix: on managed profiles, require finality before confirming the registration (as mainnet already does), or report a distinct 'not finalized, retry' code.

The Counter reference compiles under the managed settings with no forbidden opcodes, no immutables and no CBOR trailer, so the reference is clean for the inspection step. The review is complete and the findings file is final.
Scope covered. I ran the Math Precision, Boundary and Numerical Gap passes over the managed-contract delta: the candidate registry and proxy, the Counter reference, the managed evidence, module, ABI, chain-read, chain-executor, Claus controller, transaction and admission code, and the approval flow. The existing nine Foundry tests pass with the corrected allow_paths. The prototype kernel fixes hold on inspection: creation-block activation is enforced with a strict block comparison, the registry and all managed proxies are rejected as implementations at both registration and approval, the seven proxy selectors are rejected in module ABIs, and the dormant proxy cannot run module code before activation because its implementation lookup reverts at version zero.
Findings written to .imd-findings.json (4 total, none critical or high, so no proof files were required):
- Medium, proposal expiry arithmetic. The off-chain expiry is fixed at proposal creation as the larger of 24 hours or delay plus 2 hours. For any accepted delay of 22 hours or more the on-chain queue must therefore be mined within 2 hours of creation, and that same 2 hours is consumed by the Claus review itself. The controller's 300-second margin lets a late review be signed with an execution window of roughly 250 seconds, five days later. A single 15-minute retry at that instant expires the approval. Verified by arithmetic and by a passing scratch Foundry test that reproduces the revert boundaries.
- Low, finality versus confirmation. Managed evidence reads the registration target at the finalized block while Sepolia registrations confirm after three blocks, so the first proposal attempt for about 13 minutes fails with an error that claims the registry changed.
- Low, delay-bound mismatch. The registry accepts delays up to 365 days and never lowers them, while the network rejects anything above 5 days, so one direct owner call permanently removes a project from the reviewed upgrade path. Reported as a retained-design gap to document or align.
- Info, Counter readiness. An empty committed configuration is allowed, and the Counter's initializer has no access control, so the first outside caller would set the initial count. Storage layout across Counter and CounterV2 is sound.
Not found. No overflow, truncation, downcast, rounding or scale defects in the registry arithmetic, the gas reservation and settlement accounting, or the timestamp and block comparisons. Gas budgets hold the invariant that settled cost never exceeds the reservation because the receipt is checked against the signed gas limit and fee cap.
Retained design assumptions, not defects. Zero mandatory upgrade delay, the recovery owner's generation bump even without a pending recovery, permissionless queue and finish-recovery calls, and the proxy exceeding the 2300-gas stipend are all documented trust boundaries and were left as such.

The review is complete and the findings file is written and validated. The working tree is clean apart from the findings file.
Outcome. The corrected managed-contract candidate holds up on-chain. I found no exploitable fault in the registry or proxy. The one medium finding is an operational coupling from the dormant managed profile into the live readonly worker. The rest are low-severity integration and reference issues, plus two informational entries recording verified fixes and retained trust assumptions.
Findings written to
.imd-findings.json:- Medium, runtime/chain-worker.mjs:22. The worker role check now reads both readonly and managed capability records per chain and demands the single per-chain key match every record, even one with enabled false. Writing a managed capability with its own signer makes the live readonly controller and executor throw on every tick. Reproduced against an in-memory store.
- Low, reference/managed-counter/Counter.sol:9. The reference initializer has no caller restriction. With an empty committed activation, which the pipeline and its own test allow, any address can initialize count to the maximum and permanently brick increment. Reproduced with a Foundry probe through the real executor and proxy.
- Low, src/review-flow/index.mjs:67. Review requests without a chain are now persisted as Sepolia readonly. If the project deploys to mainnet before accept, acceptance fails with a chain mismatch until the quote expires.
- Low, src/review-flow/index.mjs:75. A review request naming the managed profile returns an existing readonly-labelled case for the same build, since chain and profile are not part of the case identity.
- Info, docs/managed-contracts.md:34. Zero delay, recovery-owner powers and revocation not pausing the installed module are verified as documented trust boundaries. No fix required.
- Info, ClausManagedExecutor.sol:211. The prototype kernel findings are fixed: creation-block guard, nested managed targets, registry as module, selector reservation by 4-byte selector, and the Foundry allow_paths. Extra probes confirmed no owner-signature replay across registries and no owner powers from a reentrant initializer.
Verification run. The proposal Foundry suite passed 9 of 9. The targeted JS tests for the delta passed 36 of 36 after installing dependencies. The scratch probe was removed. No proof files were attached because nothing reached high or critical severity, and the reference Counter cannot be imported from the proposal Foundry project without touching its configuration.
Not covered. I did not run the whole JS suite or the readonly contracts suite, and I did not inspect docs/openapi.json for parity with the route schema.

Retained design assumptions: zero mandatory delay, recovery-owner powers and non-pausing revocation are documented trust boundaries, not defectsdocs/managed-contracts.md:34
Verified against the contract: MIN_UPGRADE_DELAY is 0 and registration-time minDelaySeconds can only be raised (setPolicy rejects decreases); readyAt = max(now+minDelay, notBefore) so a zero-delay approval is executable in the same block it is queued; setAuthority(false), proposeProjectOwner, setPolicy and cancelApproval invalidate pending approvals through generations/nonces but nothing in the registry or proxy blocks ordinary calls to the installed module; scheduleSignerRecovery pauses registration, queue and execute via pendingSigner and finishSignerRecovery replaces the signer permissionlessly after recoveryDelay (1..365 days); cancelSignerRecovery bumps clausGeneration even when no recovery is pending.
The native-review scope text and this document state these boundaries. No fix required; recorded so the requester's explicit zero-delay choice is not re-litigated.
Owner registers with minDelaySeconds=0; Claus signs an approval with notBefore=now; relayer calls queue then execute in the same block: upgrade installs with no intervention window (as designed, covered by proposals/managed-contracts/ClausManagedExecutor.t.sol testStatePersistsAcrossExactApprovedUpgrade). Owner later calls setAuthority(id, mandate, false): a further execute reverts InvalidApproval, but Counter(target).increment() still succeeds.

.imd-findings.jsonand the pinned.imd/inputs.Outcome: no critical or high defects found in the managed-contract candidate. The on-chain kernel fixes from the prototype review are all present and verified against the code and the proposal's Foundry suite (9 of 9 tests pass):
- Creation-transaction selfdestruct:
executereverts whileblock.number <= initialModules[id].createdAtBlock, and the compiler profile's runtime opcode scan also rejectsSELFDESTRUCT,DELEGATECALLandCALLCODE. - Cross-project proxy implementations and registry-as-module:
registerProjectand_validaterejectaddress(this),isManagedTarget[...], andimplementation == target. The offchain validator additionally rejects modules with constructor arguments or immutables, so a compiled copy of the proxy cannot be admitted. - Fixed selectors:
validateManagedModulerejects any module ABI entry whose 4-byte selector collides with the seven reserved proxy selectors, and this validator runs at review admission, deploy, worker deploy, and previous-source capture. - Foundry allow_paths: the proposal's
foundry.tomlnow resolves../../contracts/liband compiles cleanly. - Registration target from receipts: the executor reads
projects[projectKey].targetat the receipt block (also for relayed registrations) rather than predicting the address. - Typed domains and profile binding: offchain EIP-712 types match both on-chain typehashes byte for byte, the managed domain name differs, and the native review prompt sends the full canonical input including the previous build, storage layout, source files and current configuration.
Findings recorded (6), most severe first:
- Low
src/chain-deployments/index.mjs:61. A review case recordschainId/profileat request time, butdeployModuleandapprovals.createnever compare that scope with the deployment's profile. Under a pilot sponsorship, which skips the profile validator, a case scoped to readonly can back a managed module deploy. - Low
runtime/chain-worker.mjs:18. Both profiles are forced to share the per-chain Claus signer and payer key. A managed capability published with a different signer makescheckChainWorkerRolesthrow and stops the whole worker, including the live readonly profile. - Info
reference/managed-counter/Counter.sol:9.initializeCounterhas no caller restriction. If activation commits an empty configuration, as the managed-chain test does, anyone can initialise the proxy afterwards. Confirmed with a Foundry test in a temporary project outside the repo. Harmless for Counter but worth stating in the lifecycle documentation. - Info retained design assumptions, documented as trust boundaries rather than defects: zero mandatory upgrade delay (
MIN_UPGRADE_DELAY = 0), recovery owner liveness powers, and upgrade revocation not pausing the installed module.

Retained design assumption: zero mandatory upgrade delay removes any guaranteed owner intervention windowproposals/managed-contracts/ClausManagedExecutor.sol:23
Explicitly owner-requested; not a defect. With minDelaySeconds = 0, queue() sets readyAt = max(block.timestamp, notBefore) (line 197) and execute() accepts in the same block, so anyone holding a Claus-signed approval can queue and execute in one transaction. setAuthority(id, hash, false) and cancelApproval(id, nonce) only help if they are mined before that transaction.
Documented in docs/managed-contracts.md ('With a zero upgrade delay, revocation has no guaranteed intervention window'). Owners may opt into a delay at registration and setPolicy can only raise it. Keep as documented trust boundary; no code change requested.
State: project registered with minDelaySeconds = 0; Claus has signed approval A (notBefore <= now).
Input: a single transaction (or two back-to-back) calling queue(A, sig) then execute(A, config, sig).
Expected under a delayed model: the owner has a window to call setAuthority(false).
Actual (by design): readyAt == block.timestamp, execute succeeds immediately; a later setAuthority(false) cannot undo the installed version.
Retained design assumption: recovery owner can suspend all registrations and upgrades indefinitely and rotate the Claus signerproposals/managed-contracts/ClausManagedExecutor.sol:279
The immutable recoveryOwner is a trusted root. scheduleSignerRecovery sets pendingSigner, which makes registerProject (line 172) and every _validate (line 308) revert until finishSignerRecovery or cancelSignerRecovery; it can be re-scheduled repeatedly to extend recoveryReadyAt, and each schedule/cancel/finish increments clausGeneration, invalidating every outstanding Claus approval.
After recoveryDelay (>= 1 day) anyone may call finishSignerRecovery, after which the new signer can approve upgrades for every project on the registry. These are the documented liveness powers ('The immutable recovery owner can pause new approvals and replace the Claus signer after the fixed recovery delay'). No code change requested; the operator should publish who holds recoveryOwner and the configured recoveryDelay in the admission evidence.
State: any registered, enabled project with a queued approval A.
Input: recoveryOwner calls scheduleSignerRecovery(X).
Expected/actual: execute(A, ...) reverts InvalidApproval (pendingSigner != 0 and clausGeneration changed); registerProject for any new project reverts InvalidRegistration; after recoveryDelay, finishSignerRecovery() installs X as clausSigner and all approvals must be re-signed by X.
Retained design assumption: upgrade revocation does not pause the installed moduleproposals/managed-contracts/ClausManagedExecutor.sol:227
setAuthority(id, hash, false) only makes _validate reject new approvals; ClausManagedProject.fallback/receive keep delegating to the current implementation because _implementation() reads moduleCode(), which is untouched by authority or policy changes. docs/managed-contracts.md states this ('Revoking upgrade authority does not stop calls to the currently installed module... the registry is not an emergency pause').
This is consistent with the requested product; any application-level pause must be part of the reviewed module. No code change requested.
State: project at version 1 with an installed implementation.
Input: owner calls setAuthority(id, mandateHash, false), then any caller invokes a mutating module function through the proxy.
Expected under an 'emergency stop' expectation: revert.
Actual (by design): the call is delegated and succeeds; only queue/execute for new versions revert with InvalidApproval.

Outcome. executenow rejects any block at or before the registration block, and no module code runs at registration. - Cross-project proxy as implementation: both

- Fixed selectors:
validateManagedModulerejects the seven reserved selectors by 4-byte value, and the proxy dispatches them before delegation. - Foundry allow_paths: the proposal project compiles and all nine tests pass with the current configuration.
- Target address comes from the
projectsread at the confirmed receipt block, not from prediction.
Findings written
- Low, Counter reference: with an empty committed configuration, which is exactly what the node reference test models,
initializeCounteris open to the first caller after activation. A stranger sets the count to the maximum and every increment reverts forever. A Foundry proof undertest/scratch/fails on current code. - Low, module_deploy: the review case now records its chain and profile, but
deployModulenever compares them with the deployment scope. Under a restricted pilot a managed build reviewed under the default readonly scope can be deployed as managed mainnet. Admission is still enforced, so the harm is mismatched published evidence. - Low, chain executor: configuration is accepted case-insensitively, hashes and encodes correctly, but the post-execution strict string compare fails on upper-case hex. The upgrade goes live on chain while the proposal retries every minute forever and blocks new proposals.
- Info: a reverted execute leaves a publicly relayable approval live until expiry while the API marks the proposal failed, and the API exposes no cancelApproval.
- Info: the worker role check forces one signer and one payer key across readonly and managed profiles on a chain.
- Info: the kernel accepts an EIP-7702 delegation designator as an implementation. A scratch Foundry test confirms registration succeeds. Only the off-chain inspector rejects the prefix.
Retained design assumptions, not defects. Zero minimum upgrade delay with opt-in longer delay, recovery owner able to pause all admissions and replace the signer after the fixed delay, upgrade revocation not pausing installed modules, and the generation bump on cancelling a non-pending recovery are all documented in the candidate docs and behave as described.
Limits. The repository has no installed node_modules, so the upper-case configuration case was reasoned from viem semantics rather than executed. I did not run the whole node suite, only the proposal Foundry suite and two scratch Foundry tests.

registerProject and _validate only require implementation.code.length != 0 and codehash == committed hash. A delegated EOA after EIP-7702 (Prague is live on both supported chains) has 23 bytes of code 0xef0100||address, so it passes both checks and the proxy will DELEGATECALL into it, executing the delegate target's code.
The account owner can re-delegate at any time; the codehash check then turns every call into InvalidModule (a liveness grief), and before that the designator does not pin the behaviour reviewed. The API never proposes such an address (modules come from network-deployed chain-modules records) and adapters/chain-evidence.mjs inspectCode rejects the 0xef0100 prefix, so this is defence in depth only.
Rejecting codes starting with 0xef0100 (or any EXTCODESIZE == 23 with that prefix) in _validate/registerProject would make the kernel self-sufficient.

For delays of 22 hours or more, the Claus review, signing and queueing must all finish within 2 hours of proposal creation. The time left to execute after the delay can then be only seconds. runtime/chain-worker.mjs:21). If a managed record is configured, even disabled, with a different signer or payer key, every worker cycle stops withworker_role_mismatch. Readonly work then stops too. Reproduced in Node. - Low – the review's recorded chain/profile is never checked at deployment (
src/chain-deployments/index.mjs:61). A module can be deployed as managed using a review case recorded as readonly, and a repeated review request returns the old readonly case. The deploy-time module check and managed admission still run, so this is a mismatch in the evidence, not a way around those checks. Reproduced with a Node harness in which the managed admission check was stubbed out. - Low – mixed-case configuration hex leaves a proposal stuck in "executing" (
adapters/chain-executor.mjs:279). The code compares the on-chain value, which comes back lower-case, with the stored string as an exact match. Reproduced the lower-case decoding in Node. - Low – the on-chain and off-chain delay limits disagree (
adapters/chain-read.mjs:127). The contract lets the owner raise the delay up to 365 days and never lower it, but the network refuses any proposal over 5 days. One direct owner call therefore removes the project from network-managed upgrades for good. Reproduced by reading the code only. - Low – the first proposal fails for about 13 minutes after a Sepolia registration (
adapters/managed-evidence.mjs:21). The registration counts as confirmed after 2 blocks, but the evidence read uses the finalized block, where the new contract does not exist yet. The problem clears once that block is finalized. Reproduced by tracing the code path; I did not run it against a live chain.
Info – design choices the owner asked for, recorded so they are not reopened
- Zero mandatory upgrade delay.
- The recovery owner's power to pause approvals and replace the signer.
- Revoking upgrade rights does not stop calls to the installed module.
- After a reverted execute, anyone can retry it with the same approval until it expires.
- EIP-7702 delegated accounts are rejected only off-chain, not by the contract.
Info – earlier prototype findings verified fixed: module code running in the creation block, other projects' proxies used as implementations, the registry used as a module, fixed selectors, and Foundry
allow_paths.Dropped: one specialist lead, that a review case defaults to Sepolia and can then never be accepted after a mainnet deployment. I couldn't reproduce it in the time available.

On-chain setPolicy allows delays up to 365 days and is monotonic, but proposal_create rejects delays over 5 days: one owner call irreversibly removes the project from reviewed upgradesadapters/chain-read.mjs:127
Owner calls setPolicy(projectId, keccak256('policy-2'), 432001): succeeds (432001 <= 31536000 and >= 0). proposal_create -> chain.proposal reads state() minDelaySeconds = 432001 -> requireThat(s.minDelaySeconds <= 432000) throws 'unsupported_delay'. setPolicy(projectId, v, 432000) now reverts InvalidPolicy (delay < p.minDelaySeconds).
Expected: bounds agree or documented; actual: permanent exclusion.
Sepolia managed binding is written after 2 confirmations but managed evidence reads at the finalized block, so the first proposal fails for ~13 minutesadapters/managed-evidence.mjs:21
Fix: on managed profiles, require finality before confirming the registration (as mainnet already does), or report a distinct 'not finalized, retry' code.

registerProject runs no module code and execute refuses the creation block (no delegatecall in the proxy's creation transaction, so EIP-6780 SELFDESTRUCT cannot delete it). isManagedTarget rejects other projects' proxies as implementations at registration and approval; implementation == registry rejected in both; proxy _implementation rejects itself/executor.
Reserved proxy selectors are compared by 4-byte selector in adapters/managed-module.mjs. proposals/managed-contracts/foundry.toml allow_paths = ['../../contracts/lib'] compiles.
Note: the proposal Foundry project cannot import reference/managed-counter (outside allow_paths), so the Counter reference is not exercised by the kernel suite.
